Memorial Medical Center, Golden Valley Health Centers Partner to Implement ED Navigator Program

Latest in a series of efforts to help patients receive “Whole Health Services”

Modesto, Calif. – Memorial Medical Center, part of Sutter Health, has implemented an Emergency Department Navigator Program to help with efforts to ensure area patients are receiving health care in the right place at the right time.

The program is in place via a unique partnership with Golden Valley Health Centers (GVHC) — a Federally Qualified Health Center (FQHC) with clinics throughout the Central Valley.

GVHC navigators, medical assistants and employees are working closely with staff from Memorial’s emergency department, care management, admitting, and insurance enrollment teams to help connect qualifying patients with primary care physicians, follow-up appointments, and essential social services.

“What we’ve found is that ED Navigators help us to better address care for the whole person in our busy emergency departments,” said Holly Harper, director, external affairs for Sutter Health’s Valley Area.

“ED Navigators have proven valuable and very effective in connecting people with resources beyond the walls of our hospitals,” said Harper.

“The collaboration with Memorial allows us to help patients find a medical home so that they don’t utilize the emergency department for non-emergency visits but can still get the care they need,” adds Ellen Piernot MD, MBA, Golden Valley’s chief medical officer.

Sutter Health has also been expanding the number of Urgent Care centers and Walk-In Care clinics to provide more options for Northern Californians to receive care in appropriate settings.

“Sutter Health is focused on access to the complete continuum of care options, including primary care, urgent care, Walk-In Care clinics, hospital care, and even video visits, so all patients receive the care they need, when they need it,” said Harper.
