Patient-Centered Medical Home Pilot Celebrates One Year

The Patient-Centered Medical Home (PCMH) team based in the Sutter-affiliated Davis Family Medicine Care Center celebrated its first anniversary this month. The PCMH pilot, a joint effort between Sutter Health and Sutter Medical Foundation, aims to improve the care and service that patients receive in a way that enhances health outcomes, increases overall efficiency and reduces total health care costs.
PCMH is built upon a team-based approach to patient care. In this model, a multi-disciplinary care team supports patients’ health care needs at the right time, in the right place (even outside care center walls) and by the right team member. This approach helps providers proactively anticipate their patients’ health care needs, while involving patients more in planning and coordinating the care they receive.

Two additional Sutter Health PCMH pilot sites launched earlier this year in Albany and Santa Rosa.
