New Partnership Expands Healthcare Services to Seniors

Integrated PACE Services Coming to Stockton and Modesto Areas

WelbeHealth and Sutter Health have announced a partnership to expand Programs of All-Inclusive Care for the Elderly (PACE) offerings to new communities in the Stockton and Modesto areas, giving thousands of medically frail California seniors new access to comprehensive services that can keep them healthy and living independently in their homes and communities, rather than confined to hospitals, nursing homes, and other facilities.

Stockton is the largest city in California without PACE, an innovative and proven national model for holistic care that brings health care, social support, transportation, and insurance under a single care team focused on ensuring the highest quality of life for seniors they serve.

Sutter Health has operated one of the original PACE programs, based in Sacramento, for the past 25 years and, after many months of thoughtful discussion, has engaged WelbeHealth to launch and operate new PACE programs.

PACE makes a major impact on the quality of life for medically frail seniors and is especially important to communities around the U.S. at a time when more than 10,000 baby boomers are aging over 65 each day. The new partnership estimates that several thousand PACE-eligible seniors currently reside in the Stockton-Modesto area.

The Program of All-Inclusive Care for the Elderly (PACE) is a value-oriented model of care that combines Medicare and Medicaid into a single flat-rate payment to cover a comprehensive set of health and personal care services. From hospital visits to transportation, PACE programs provide a full range of preventive, primary, acute, and long term care services so that aging seniors—nearly half of whom are diagnosed with Alzheimer’s or a related dementia—can continue living comfortably and safely in the community. Utilizing a coordinated, team-based care model, PACE program are able to serve medically frail seniors who are on average 77 years old, suffering from 15 medical conditions, and impaired in multiple activities of daily living.
