Sutter Health’s AIM Program Featured for Excellence in National Publication

Sutter Health’s Advanced Illness Management (AIM)® Program recently earned a spot in CAPG’s 2015 Case Studies in Excellence publication. CAPG’s annual report features case studies from among the country’s most innovative healthcare organizations.
The case study titled “Sutter Health’s AIM Program: Making a Difference in the Lives of Patients and their Families,” highlights AIM as a successful example of risk-based coordinated care and an example of how health providers can successfully improve systems to drive better quality and better care experience for patients.
AIM, available in 19 counties across Northern California, provides highly personalized, home-based medical care and emotional support to patients with advanced chronic illness so they can feel empowered to manage their symptoms and improve their quality of life.
