Sutter Health’s Advanced Illness Management (AIM) ® Featured in National Publications

Sutter Health’s Advanced Illness Management (AIM) ® program provides compassionate, personalized health services to aging individuals throughout Northern California. Featured recently in two national articles – Brookings: Opportunity Knocks: How Home-Based Primary Care Offers a Win-Win for U.S. Health Care and The Atlantic: The Hospital is No Place for the Elderly –  AIM’s multidisciplinary, home-based approach reduces hospitalization and improves care transitions, contributing to an improved quality of life for participants enrolled in the program.

Sutter Health recognizes that all patients—especially those at the end of life—deserve a health care partner who takes time to understand their personal health concerns, as well as their personal preferences and goals. With primary oversight from a patient’s physician, AIM nurse managers partner closely with patients and families. AIM’s innovative program offers palliative care and advance care planning so that patients with advanced chronic illness can better manage their health and live the lives they want.

With a multi-year AIM pilot program showing positive outcomes, in 2012 the Center for Medicare & Medicaid Innovation (CMMI) awarded Sutter Health a three-year, $13 million Health Care Innovation Award to support the expansion of AIM throughout Northern California. Through the grant and expansion, AIM enrolls nearly a thousand patients across the Sutter Health network. Results from the AIM pilot program:

  • Reported a 75 percent reduction in days spent in an ICU at 90 days post-enrollment.
  • Resulted in a more than 60 percent reduction in hospitalization.
  • Showed extremely high patients and family satisfaction (4.7 out of 5).
  • Reduced costs for payers by $5,000 per patient at 90 days post-enrollment.
